Usually operator error.
While futzing around in the rain I'll tag "video" just as I slide the phone in my pocket...video my feet, sidewalk, sky, etc.
It's boring inside a shirt pocket. Just various shades of 'dark'.
And night shooting leaves the camera open a bit longer....which means I need to hold it still, longer.

Good that you persevere though Stig.  The last photos I took for this forum were only 50% usable!  Two OK while two were very definitely too blurry to post.  I suspect in my haste (parked on a 60mph single lane carriageway!) I didn't hold the phone still enough.
