Found one on Amazon (actually came from Amazon Global UK) - $75.27 delivered.

Installation took about an hour - I just followed the diagram and hand-tightened everything.  Then I snugged everything up, made adjustments, and tightened everything down.  With no actual instructions, I'm almost certain I'll be adjusting something later on but right now it looks right.  ???
